Androgenic alopecia (AGA) OR Male pattern baldness (MPB)
Male pattern baldness is most common type of hair loss in male. This type of hair loss is also known as genetic balding. Thinning of the hair on entire scalp, hair loss on temples or the crown of the head could be sign of male pattern baldness. Male pattern baldness is also known as androgenic alopecia.
In some patient typical pattern can be seen but in some cases, generalized thinning can be seen which is known as diffuse patterned alopecia. In diffuse patterned alopecia thinning can be seen on front, top, and vertex of the scalp but back side of the hair (permanent zone) can be stable. In some cases, thinning can be seen on entire scalp including permanent zone which is known as diffuse unpatterned alopecia.
The most widely accepted explanation for male pattern hair loss is the DHT (dihydrotestosterone),which is a by-product of testosterone created by the enzyme 5 Alpha-Reductase. Medical science believes that overproduction of the male hormone or androgen called DHT is an important cause of hair loss in men however it cannot be the only cause. DHT is a natural form of testosterone produced by an enzyme called 5-alpha-reductase which is required for normal sexual functions. Indiscriminate inhibition of this metabolite has led to sexual performance problems, such as impotence or erectile dysfunction. In most cases, male pattern baldness / androgenic alopecia is known for permanent type of hair loss where the chances of re-growth are poor with any medical treatment. There are thousands of commercial products available in the market but most of them are known as placebo treatments. Conventional doctors are using topical minoxidil, finasteride, vitamin pills, laser treatment, PRP treatment etc. However modern treatment does not give permanent recovery in such type of hair loss and it has side effects too. Minoxidil has been proved less effective and it has some adverse reactions like burning of the scalp, erythema, scaling, pruritis, dermatitis, tissue damage etc. Finasteride has adverse effects including impotence, abnormal ejaculation, abnormal sexual function, gynecomastia, erectile dysfunction and testicular pain. PRP treatment is too costly and also gives temporary re-growth only.
